Amen Clinics Intake Forms
The forms below are used in the intake process and are available for download. Call the clinic to make an appointment and bring your form, filled out completely, with you for your appointment.
Download Instructions: Choose either the Adult Intake forms or the Child/Teen Intake forms from the links below.
Print the forms, fill them out completely. (be sure to print your name at the top of each page in the box provided).
Adult Intake Forms - click to download the Adult forms in MS Word document.
Child/Teen Intake Forms - click to download the Child/Teen forms in MS Word document.
If you are using a IBM compatible PC and you have Microsoft Office, you can open these files in MS Word and print them.
If you are using a IBM compatible PC and you do not have Microsoft Office or Microsoft Word, you will be prompted to download the files to your computer. When the download prompt comes up, select "Save this program to disk". You will then be prompted to select the location that you want the file saved to, browse to and select your desktop. To view this file, open it in Wordpad.
If you are using an Apple computer (Imac, G3, G4), and you have Microsoft Office, Microsoft Word will open these files in a new window.
If you are using an Apple computer (Imac, G3, G4), and you do not have Microsoft Office, download the files to your desktop and open them with Simpletext.
